NGAL，也称为致癌基因24p3或脂质运载蛋白-2（LCN2）通过螯合铁参与先天免疫，进而限制细菌生长。它在嗜中性粒细胞中表达，在肾脏，前列腺和呼吸道和消化道的上皮细胞中表达水平较低。
在急性肾损伤（AKI）的情况下，NGAL在受伤后2小时内以高水平分泌到血液和尿液中。因为NGAL具有蛋白酶抗性且很小，所以蛋白质很容易在尿液中排泄和检测。AKI患者的NGAL水平与其预后的严重程度相关，可用作AKI的生物标志物。NGAL还可用作慢性肾病，造影剂肾病和肾移植等手术的早期诊断。
肾脏健康常见的是血清肌酐。血清肌酐是肾功能的标志物，而NGAL是肾损伤的标志物。NGAL水平是诊断AKI的更和敏感的标志物，而不是血清肌酐水平。事实上，已经证实NGAL尿排泄的增加是由于在通过其他方法检测到任何损伤之前发生的管状改变。因此，监测NGAL水平可减少延迟的AKI诊断和治疗。使用更敏感和特异的标记物可以更早地诊断，正确应对AKI，降低发病率和死亡率。
